COVID-19 WAY OF WORKING
PLEASE ATTEND YOUR APPOINTMENTS AS USUAL.
PLEASE FILL IN ALL YOUR FORMS USING OUR PATIENT PORTAL BEFORE YOU ARRIVE FOR YOUR APPOINTMENT.
There are 3 parts to complete:
1. Your personal details
2. Medical History Form
3. COVID-19 Questionnaire Form
If you have travelled in the last 14 days before your appointment please notify us.
Please can you fill out the COVID questionnaire AGAIN on the day of your appointment.
(Click the menu on the side to access the COVID-19 section – once logged in)
This is to make sure that there have been no changes to your COVID-19 status. It will also minimise the time you spend in the practice for your safety.
Please note that you will have to log off and log in again when completing forms for each member of family.
Thank you for taking the time to fill these out. Due to the COVID-19 pandemic we have to plan all appointments carefully for your safety.
The door will be locked when you arrive. For your safety and for ours, we are only letting patients into the practice for their appointment only.
Please arrive on your own to reduce the number of patients in the practice. If you are accompanying your child for their appointment, only bring the child/children that have appointments.
Please wear a face covering. If you don’t you may be refused entry into the practice. Alternatively, you may purchase one from reception before entering.
Check you have filled out the Medical History Form, COVID-19 Questionnaire and check-in for your appointment online via the Patient Portal
Call us to let us know you are outside or wait in your car. If the telephone line is busy then please wait and a member of staff will come to the door.
We will let you into the practice after taking your temperature and providing hand sanitising gel.
Please leave your belongings in the box by reception. We kindly ask you to bring as little things as possible to reduce to spread of infection.
When the clinic room is ready we will take you through.
We will be wearing more PPE than usual, so we’ll look a little different.
At the end of your appointment please sign any necessary documents on our tablets. We will open the door for you just as you leave the premises.
